What Is Calcific Tendinitis?

Calcific tendinitis is one of the most painful shoulder conditions and causes unbearable shoulder pain. It is characterised by calcium deposits within the tendons of the shoulder's rotator muscles. The exact cause is unknown. It is more common between the ages of 40 and 60, especially in women. Diabetes is known to increase the tendency. Apart from this there is no known major risk factor.

What are the signs of calcific tendinitis?

The main complaint is severe pain. In the acute (early) stage, patients complain of severe shoulder pain, especially at night. Raising the arm to the side, or even moving it at all, is very painful. Patients usually keep the arm still and close to the body. This stage lasts about two weeks on average. In the chronic stage, continuing pain and immobility lead to weakening of the muscles around the shoulder.

How is calcific tendinitis diagnosed?

Seeing the calcium deposit in the shoulder on an X-ray is enough for the diagnosis; MRI and ultrasound may also be requested to check the integrity of the tendon.

How is calcific tendinitis treated?

In the acute stage the main aim is to control the pain. The shoulder is rested with the arm in a sling. Painkillers and cold application are recommended. Once the pain eases, physiotherapy is started. ESWT (shock wave therapy) can be used.

In patients who do not respond, needling under ultrasound or fluoroscopy can be performed to break up the calcium deposits in the shoulder.

In patients who do not benefit from needling or who also have a tendon tear, the calcium deposits are removed with open or closed (arthroscopic) surgery.
